Case #1: The patient is a 12-year-old with a past medical history of status epilepticus and shunt revisions.  Currently hospitalized for cranial surgeries.  The patient’s caregiver is a single parent who was previously working a full-time job but, due to current admission needs, has been unable to work.  The unexpected loss of income has affected the parent’s ability to pay all of the utility bills ($330.05).  The family is also having to move to a more wheelchair-accessible home due to the patient’s continued weakness/assistance required for ambulation.  The parent was able to find accessible housing but needs assistance to pay the security deposit ($318) and the first month’s rent ($220).  The apartment will be available in June, but the patient is expected to continue hospitalization for acute care rehabilitation.  Appropriate housing will need to be secured for a safe discharge.

Case #2: The patient is a 34-week preterm baby who has been hospitalized at ACH for over six months with a respiratory disease requiring a tracheostomy.  The parents recently moved into an apartment after being homeless in preparation for taking the patient home.  The family will continue working for future needs, but they are in need of a crib, mattress, and organizer for the patient’s medical supplies for a safe discharge.
