Case #1: This patient is a complex trach and vent-dependent child who is being followed by a multidisciplinary team. The patient has been admitted for over six months and is scheduled to continue inpatient admission
for another 6 months to a year with the goal of decreasing vent dependency and for caregivers to learn how to care for them. When patients are discharged, they will need 24-hour home care, which will likely require
one of the caregivers to have to quit their job to become a full-time caretaker. Caregivers currently have employment without benefits and inconsistent pay and working long hours, which requires them to
sacrifice time to see patients. Caregivers have attempted to utilize other social support options in addition to working to improve the current situation but are still struggling. We are requesting one-time financial assistance need to pay $319.98 towards helping the family keep their electricity.

Case #2: This patient is a trach/vent-dependent child who has been inpatient for over five months. Caregivers need to be at the bedside regularly for the trach/vent training for the goal to be discharged home. This admission has affected caregivers’ income, causing financial struggles towards making rent and utility payments. We are requesting $520.35 toward utilities: water ($83.59), gas ($436.76), and $800 for rent.

Case #3: This case involves multiple children who have been diagnosed with Cystic Fibrosis. Additionally, one of the patients was also diagnosed with a hernia, which will require surgery and hospital admission. These medical needs have affected caregivers’ financial abilities, which was further affected by Dad being denied of paternity pay leave. We are requesting assistance for one month’s work of utility, auto, and mortgage assistance while caregivers take care of doctor visits and hospital stays.

Case #4: This patient was a highly complex case and was followed by a multidisciplinary team. Caregivers were informed patient would never be able to breathe or eat on their own or meet any developmental milestones. Caregivers ultimately decided to place the patient on comfort care and withdraw support. Due to the vast nature of the situation, the mother has been unable to work consistently for several months. Her employer does not offer paid leave. The mother required a C-section due to the patient’s medical issues and was re-hospitalized on two separate occasions. Mom returned to work for two weeks before the family needed to be present at the hospital for the patient’s last days of life. Mom will likely not be able to return to work for another one or two weeks. We are requesting two months of rent assistance.

Case #5: This patient was born prematurely, which has caused breathing and feeding issues, and has needed surgery for a bowel obstruction. This has been inpatient for over a month, and due to the complex nature of their case and being followed by a multidisciplinary team, they cannot establish a timeline for discharge. The family is a one-income household, and due to the needs of the patient, the dad has been unable to bring in income, which has strained them financially. We are requesting assistance for one month’s mortgage to provide family time to identify alternatives to making income.

Case #6: This patient was born prematurely, which has caused breathing and regulating glucose in addition to needing a nasal gastric tube for feeds. Pt. is also undergoing a workup for increased heart rate issues. Pt. is expected to be hospitalized for a minimum of 6 more weeks. Additionally, mom (who was the only full-time employed caregiver) incurred some complications from the C-section as well as a separate injury prior to birth. Mom is not cleared to return to work until released by her PCP (eta May 28th). The family has been using tax returns and what they had in savings to pay bills, but they are concerned about not making mortgage payments for the next few months. Mom is confident once she returns to work, to be able to cover the household bills again. We are requesting assistance with three months' mortgage payments to assist until the family can get back on their feet.
