Case 1: This patient has a diagnosis of Down Syndrome and has severe cardiac complications. Due to the patient’s fragility and diagnosis, they are unable to attend daycare. This has caused the mother to have to quit her job to be the full-time caregiver for the patient. The family has applied for SSI and food stamps to assist with this financial hardship. The family was preparing to transfer to comfort care and was not financially prepared for this hospitalization. The request for this family is $106.65 for water, $94.03 for electricity, $187.80 for gas, and a $945 mortgage for a total of $1,333.48 to assist the family from getting behind on their necessities while they wait for SSI and food stamps. After the family starts receiving these resources, they are confident between this and the father continuing to work full time that, they will be able to cover their monthly expenses.

Case 2: This patient was diagnosed with a chronic pain condition in December 2022. The patient has had to be admitted multiple times since the diagnosis and has four other siblings at home. The mother is staying at the hospital with the patient, however, the father has to stay home with two of the four siblings who are not yet school-aged. This has had a negative impact on their income and has caused the family to get behind on their electric bill. The request for this family is for $642 to pay for two months of electricity and get their account up to date.

Case 3: This patient is a Hematology/Oncology patient who requires monthly visits to ACH from approximately 50 miles away. The patient’s father is disabled, and his mother is working two jobs to provide for the family. Taking off work monthly to come to appointments had a great impact because the family was already struggling and regularly faced food insecurity. The request for this family is $115 for water, $205 for electricity,ity and $971 for mortgage, for a total of $1,291.

Case 4: This patient has kidney failure and receives treatment twice a week. The goal for the patient is to work toward a kidney transplant. The family lives in a rural area without access to clean water and regularly uses a creek for their water needs. In order to continue towards the goal of a kidney transplant, the patient has to have clean water to reduce the risk of bacteria and infection. After extremely diligent work between the primary social worker, social work supervisors, and the city the patient resides in, the city has agreed to get a water connection for the residence. The cost of adding this water connection is $1,000, a W-9 for the project has already been received, and all parties are fully informed of this project. The family is unable to pay $1,000 for this and is requesting assistance from the Baby Sharon Committee in order to obtain clean water.
