Case #1: The patient is a 4-week-old born via emergency C-section due to mom’s medical issue for which mom did not survive.  Due to this situation, the patient developed a devastating brain injury that is not survivable.  In the coming days, the family will be making the decision to withdraw life support from the patient.

The patient’s father and mother were both employed full-time and attending college to earn advanced degrees.  Due to the mother’s passing, the family has lost one income, and the father has been off work to make funeral arrangements.  Father drives over an hour each day to spend a few hours with the patient and drives back to attend to other siblings.  Last week, the family’s only vehicle broke down on the highway.  The car had to be towed, and the local mechanic shop found the car needed a new transmission that would cost $3957.  Due to the care being in the shop and the family having limited resources at this time, Dad has been limited in his ability to visit a patient.  Our hope is that the committee can help this father pay for the transmission to fix his vehicle so he can be with his child before they pass.

Case #2: The patient is a 4-month-old admitted in the ICU and likely be admitted for an additional two or more months. Parents are falling behind on all bills, car needs, etc now that they are on a one-income family.  Mom is still on maternity leave from her job but is at risk of losing it since she has been hospitalized for five months.  Family’s rent is the current highest expense and would help alleviate the financial stress the most but mom is open to having car insurance/payment assistance if she can get the companies to provide documents.  Currently, we are requesting three months’ rent to mitigate some of their stress.

Case #3:  This case involves a pre-teen patient who was born with spina bifida with hydrocephalus.  As a result, the patient is unable to walk and requires a wheelchair.  Several specialists at ACH, due to their medical issues, see this patient, which requires the family to leave home frequently.  This also requires the family to take the patient out of the wheelchair and have to carry up/down steps.  As the patient continues to grow, this has become more of a safety issue for both patient and family.  The family is seeking assistance in obtaining a portable wheelchair ramp for the family home, for which Medicaid does not pay.  We are requesting $775 for this need.

Case #4: This case involves a 2-month-old who will be admitted for at least another four to six weeks.  Mom has been on unpaid maternity leave for six weeks.  This additional length in the patient’s hospitalization will prevent the mom from returning to work.  This situation has caused financial stress to the family, so we are seeking assistance for two month’s rent ($1500) and a utility bill ($158.60).

Case #5: This case involves a 2-year complex care patient, including GI and Cardiology.  The is in need of a Central Line vest, which helps keep the CVL (central line) more secure.  Since the patient is more mobile, this would be a great benefit to the patient and family.  Unfortunately, this vest is not covered by insurance, and the family is unable to afford it.
