Case #1.  This case involves a sickle cell patient who has been experiencing multiple pain crisis events.  This has made it difficult for single parent to work as often as they are used to because they need to stay home with patient.  This has resulted in reduced income over the past two months and unfortunately, caregiver has gotten behind on rent.  Caregiver also has two other children that attend daycare and have developmental delays that require treatment. Caregiver is hoping to get patient’s pain crises in better control so they can return to their second job and get back on their feet.  For your consideration, we are requesting assistance with rent for $1,250.   Total Ask:  $1,250 APPROVED

Case #2.  This case involves a recently diagnosed sickle cell patient who relocated to Northeast Arkansas from Tennessee due to high exposure to community violence.  Family is comprised of a single parent and three children.  Family was not expecting patient’s diagnosis, which requires them to travel to ACH often for treatment and follow up.  Family does not have personal transportation so they utilize Medicaid transport.  This can be unreliable at times and is not available to help with all of their transportation needs.  Parent is working as much as possible while children are in school but has to get rides to/from which can be a struggle.  Parent does not have family/friends in area and has relied on help from neighbors.  Parent has been trying to save up to purchase a vehicle and is seeking monetary assistance to help towards the purchase of a vehicle, which would be a life changer for the family.    In summary, we are requesting for your consideration $5,000 towards a vehicle.  Total Ask:  $5,000 APPROVED

Case #3.  This case involves a patient with chronic illness, which requires them to be on dialysis nightly.  Parents are currently going through a separation, which will require both parents to now be employed to manage finances.  Patient will be under mother’s care and she does not have a personal vehicle.  Family has limited resources but is doing everything they can to get patient to all appointments.  The addition of a vehicle will help mom be able to start working again and lesson the burden on other supports to her for transportation needs.  In summary, we are requesting for your consideration $5,000 towards a vehicle.  Total Ask:  $5,000 APPROVED

Case #4.  This case involves a newborn patient that was diagnosed with Cystic Fibrosis.  The family lives in Northwest Arkansas (about 3hrs from ACH) and they have other child as well.  Due to the ACH Neonatal Intensive Care Unit’s current Isolation Policy to protect the patients’ health, it does not allow the sibling to stay with family on campus.  The family has been driving back and forth to ensure consistency for the sibling who has special needs which requires specialty care appointments that must be kept.   This family has a limited support system of friends.  Family does have some savings but at this time, mom is unable to work due to recovery from labor and delivery.  Father is currently on FMLA from work so that he may also remain with patient and help with other child.  Family is asking for help with car payments for each parent ($445.74 and $531.43).  Total ask: $977.17 towards car payments APPROVED

Case #5.  This case involves a patient has been admitted since last summer for acute hypoxic respiratory failure, which resulted in the patient being trach/vent dependent.   Family has been at ACH daily for the daily training needs to prepare to go home.  Family is working part-time and are traveling frequently from Northwest Arkansas (about 3.5 hrs.) to complete training.  Due to this long-term hospitalization and training needs, family has been unable to keep up with their rent and car payment needs for the past few months.  We are requesting for your consideration rent assistance for $3001.82 and car payment assistance for $1836.05.  Total ask:  $4, 837.87 APPROVED

Case #6. This case involves a nearly month old premature patient from Northwest Arkansas and diagnosed with Cystic Fibrosis. Patient was admitted to ACH to receive specialty care for this condition.  While this care is much needed, than has unfortunately separated the patient from her parents and three siblings.  Additionally, patient is relying on mother for breastmilk, which is incredibly beneficial to premature infants and has shown to speed recovery.  The family has a limited support system and limited resources available to them.  Mom was hospitalized due to complications from patient’s birth.  Mom normally stays at home to care for other children and due to the needs of mom/patient, the patient’s father had to take time away from work to remain with children.  The family’s home is approximately 170 miles away from Little Rock.  With the current average gas prices around $3, we are estimating each trip will cost family an average of $50.00.  We are uncertain of how long the patient will remain in Little Rock but estimate that over the next three months, they will spend at least $600.00 in fuel to make the trip to Little Rock once a week.  If mom is able to bring stock of her breastmilk during weekly visits, this could be life changing for patient.  Therefore, we are requesting for your consideration $600 towards a gas card.  Total Ask:  $600.00 APPROVED

Case #7  This case involves twins seen in Complex Care.  Both patients were admitted into theNeonatal Intensive Care Unit back in the summer of 2023 which one of the twins staying until the end of the year.  While the twins were admitted, patient’s father was tragically killed.  This tragic situation resulted in mom losing all emotional/financial support from father’s side of the family.  Additionally, mother has three other siblings and her only support is her mother who is battling her own serious medical condition.  The enormity of these circumstances has caused mother to get behind on everything including rent and electricity payments.  Therefore, we are requesting for your consideration $408.02 towards rent and $209.62 towards electric bill.  Total Ask:  $617.64 APPROVED

Case #8 The patient was born premature and continues to endure significant medical challenges.  Patient has chronic lung disease among many other diagnoses and medical concerns.  The parents have fallen behind on several bills and payments; however, their primary concern at this time is ensuring that their apartment is safe for patient to inhabit upon discharge.  They have found a new apartment complex to move to and are requesting assistance with the first month's rent ($850), the security deposit ($850), and the moving truck rental ($375) totaling $2,075. The family stated that they do have a balance of $795 to move out before 5/28 which is their date for lease renewal, but stated that if the Baby Sharon Fund can pay for the moving costs, they can cover the $795. APPROVED by email on 5/7/24
